Our urology practice is unique in the manner in which we approach men's health and urologic health, both for men and women, all told we have a , a very holistic approach. We try to use more natural and nutritional based therapies in addition to surgery and medications and all the standard things you'll get from a urologist. But when you come to us with a problem, it's not just going to be a medication or a surgical procedure that you're gonna be offered. You're gonna be offered ways of behavioral therapy. Mm-hmm . <affirmative> , uh, the idea that many times when patients are diagnosed with a new cancer diagnosis or stones or anything else mm-hmm. <affirmative> that they feel is outta their control, the first thing they ask about is what should I eat?

Yeah. Every time you said, and

It, it psychologically has everything to do with about control. Well, I can't control that disease process. It sounds like it's outta outta control. Right. But what can I control? And I can control what I put in my body. It'd be nice if we did that when we're healthy, not after we get sick, but there's still things that we can do after we get sick. And so if you want to come see us as a patient , uh, want to establish care or our favorite second opinions mm-hmm. <affirmative> . That's right. So you're already under care. That's , you're happy. Not Happy's recently diagnosed with prostate cancer, kidney cancer, bladder cancer, and want a second opinion. It may be that we recommend the exact same treatment as your other doctor , surgery, radiation, whatever it may be. Mm-hmm . <affirmative> . But then at least we can layer it on with all of our nutritional and biomechanical and physical therapy and all the other things that we do here. Right. Uh , to maximize what your functional outcome's gonna look like afterwards.

<laugh> . So how your wellness, our wellness department started was back in the day when surgeons would tell patients , uh, you should really go lose 40 pounds before I do surgery. And then they would send them off into the world, didn't they? They didn't really have a plan.

Speaker 3:

That's right. Many and many of them, you know, even after treatment, whether it be for like kidney stones or anything else, like they just, there's just no follow up mm-hmm . <affirmative> on how , how they're gonna be managed. And I , I think that that management's real important. Right. Uh, your questions really drive this practice and the, and the show mm-hmm . <affirmative> . Dr. Mystery , we were about halfway through this segment, but I wanted to ask you, what were your favorite cases last week in, in office ? I love it when you talk about the actual in-office things.

Well , uh, uh, two things happened that were awesome. Mm-hmm. <affirmative> . So number one, we had a superfan. Oh,

Speaker 4:

Warren. Hello Warren.

Speaker 3:

One of our patients, <laugh> , who's listened to every show, knew I everything and his cataloged every single thing came in and he's exactly why we do the show. Right. He is someone who has a longstanding history of B P H. He had elevated p ss a , he's got other things going on with him mm-hmm. <affirmative> and helping him navigate this process in a way that helped where he understood what was going on. Mm-hmm. <affirmative> and he understood what the next steps were and making sure that he was holistically taken care of, I think is exceptionally , uh, good for him. Right. Uh, we have ways of treating erectile dysfunction that people don't , uh, necessarily utilize everywhere. Mm-hmm. <affirmative> and if, if , if the treatment's not right for you, we don't do it. Right . And that's, that's also great. But I think my favorite case , um, uh, this week was , uh, an eight centimeter kidney tumor.

Ooh . Kidney tumor. That's

Speaker 3:

Right. We had a 62 year old woman, she was sent to us , uh, because of a CT scan she had in the emergency room for abdominal pain. Mm-hmm. <affirmative> and a and a , the CAT scan found a tumor on her kidney. Oh my . Now this , uh, what was interesting is, so number one, about 90% of kidney tumors today are found incidentally. That means that mm-hmm . <affirmative> , they don't have any pain or symptoms.

Why? Why incidentally, because they had, they literally

Speaker 3:

Have no pain. A CAT scan for no rea for some other reason. So by luck, by luck in some ways it's kind of, you know, super lucky that they had this imaging done. And because if you do have symptoms of a kidney tumor mm-hmm . <affirmative> , it'll be blood in the urine or pain or feeling something in your flank, and that means that thing is super big. Oh my

God. Like

Speaker 3:

12 inches big, and now you have really little chance of surviving this kind of thing. Oh

Speaker 4:

My heaven . So

Speaker 3:

If you have symptoms of, of cancer, it could already be too late. Mm-hmm . You're beyond the curative stage. Mm-hmm. <affirmative> , uh, which, which leads me to what a lot of patients say all the time when we diagnose 'em with prostate cancer or bladder cancer or kidney cancer, the first thing they say is, well, I'm not feeling anything as if they're like, questioning that I gave them like a false diagnosis. You know, like , well, I'm not feeling anything. I don't think I should have treatment. Right . I say to them, well, once you start feeling something, things go downhill real quick. Mm-hmm. <affirmative> . So better to find these things early in the process. Right. So we perform something called a robot assisted, laparoscopic, partial kidney removal or partial nephrectomy. Mm-hmm. <affirmative>, it's a surgery. Takes about two hours of operative time. We go in, we find the kidney, we find the kidney tumor. Mm-hmm. <affirmative> , we ultrasound , uh, right there inside the body and try to determine how, how, how deep into the kidney it goes. We find the artery and the vein of the kidney and clamp them off with a clamp. Mm-hmm. <affirmative> . And then in under 30 minutes it's like, race by the clock.

Oh, wow.

Speaker 3:

Because you can't keep the thing clamped forever. Oh. You'd have to take the kidney tumor off and sew it back together.

Speaker 4:

Robotically.

Speaker 3:

Robotically.

Speaker 4:

Wow. That's , it's super cool superhero stuff.

Speaker 3:

It is super cool every time . Do you care wear a cape every time ? <laugh> and nothing else <laugh> ? Uh , it is. No, no, no. I wear, I'm fully clothed in the operating room, but , uh, it is , uh, it is , uh, one of the coolest operations that we do. Mm-hmm. <affirmative> , and this is , uh, in distinction, when I trained 15 years ago, we didn't have the robot. We didn't have Oh , right . All of this laparoscopic stuff. Mm-hmm. <affirmative> , like she would've lost this kidney. Like we didn't, we would never even would've tried to remove part of it 20 years ago, let's say we would've just booked her and taken her kidney out. That's not

Ago. That's not too long ago compared to , uh, but , but, but how science and technology has changed over the last, even just during my , uh, medical career has been amazing. Right . And so if you're out there and you've been diagnosed with a kidney tumor , um, this particular patient came to me actually as a second opinion. Oh, really? She had no insurance. She was seen in the emergency room, saw another doctor in town, that other doctor didn't feel fully confident that they could remove just the tumor and Oh . And they thought the kidney needed to go. And that

Was a

Speaker 3:

Neurologist . They , they were gonna do it with a bigger incision. Mm . And , uh, I'm so happy for her that she came to us because we were able to, you know, provide her with state-of-the-art treatment and you , she's gonna go home in two days and do Well, the tumor was removed and , uh, you know, she'll probably live a completely normal life with just missing just a little part of her kidney . What's

Speaker 4:

The downtime for that? She's gonna be in bed for a week or so.

Speaker 3:

No, you're not in bed at all. We want you to get up and walk around. Oh. Uh, and so we want you to walk around. There's a lifting restriction mm-hmm. <affirmative> , uh, that lasts for about four weeks. Hmm . But for the most part, you're gonna feel and do fine , uh, within just a few days. Wow.

That's amazing.

And that's really true of a lot of our advanced prostate , uh, and kidney robotic operations. Mm-hmm. <affirmative> , uh, people are up and walking , uh, much sooner than you think that they would be. And they feel pretty normal pretty quickly.
