Bonus Episode: ADHD and ED: A Listener Asks, "Will Stimulant Medication Help or Hurt My Sex Life?"
In this segment, Dr. Mistry and Donna Lee answer a listener's question on ED and ADHD. She writes that her 57 year old fiance was recently diagnosed with ADHD and already takes Cialis with Lexapro to help him "last longer." She wonders, "Will stimulant medications have any positive or negative effects on our sex life?" Dr. Mistry points out that ADD/ADHD medications have different effects on different people. Stimulant medications in particular can cause men to have symptoms of erectile dysfunction. While stimulants can calm and focus the minds of some people with ADD/ADHD, they can overly-excite others and cause them to feel jittery, restless, or even agitated. Perhaps counterintuitively, it's harder to achieve arousal or orgasm when your mind and body are already in overdrive. In women, these medications are less likely to amplify distractions and more likely to help them remain present and focused, thus improving the likelihood of orgasm.
For men and women, ADD and ADHD medications can have other negative impacts on urologic health. Stimulants often cause bodily inflammation and dehydration. This can cause or exacerbate kidney stone formation and bladder irritations, which can lead to urine leakage frequent or urgent urination. Dr. Mistry adds that patients who can achieve erections while taking Cialis are prime candidates for shockwave (also called sparkwave) therapy, a painless and treatment that uses focused ultrasound waves to improve ED symptoms. Make sure to obtain a urologic consultation from a physician or medical expert before purchasing shockwave or linear compression treatments, especially from pop-up "men's health" clinics that only sell one type of treatment. Speaker 1:We have some amazing things that we can do for stress incontinence that don't require surgery. Mm-Hmm . <affirmative> don't require mesh. And so no mesh if you're suffering from that,

Speaker 1:And then we are number two in two of the different categories on feed spot .

Speaker 1:And then somebody else about a urology coding website made the hugest deal that they were number two for coding. And I was like on the same list and I was like, oh, I guess somebody thinks a big deal.
Speaker 2:Is there a coding list? <laugh> ? I dunno . It sounds terribly boring.
Speaker 1:Anyway, so this , uh, conversation got really boring, but , uh, your questions are usually not boring. So why don't you go ahead and give us a question there .
Speaker 2:They're not boring. We have plenty of questions for you, Dr. Mystery , and I'd like to remind the listeners that Dr. Mystery does not know what's coming at him. And so it's always a fun surprise. This one is from a listener just a few days ago. Hello, Dr. Mystery , my 57 year old fiance recently diagnosed with ADHD . So I think he was saying he is 57. Will stimulant medications have any positive or negative effects on our sex life? He currently takes Cialis. I gotcha . It's a female sending this. He currently takes Cialis daily with five to 10 milligrams of Lexapro as well to help him last longer. What are your thoughts?
Speaker 1:The first thing is that in my experience, these kinds of , uh, stimulants can have varying effects on , uh, on men. So on
Speaker 2:Their erections.
Speaker 1:On their erections . Oh , also , uh, although not asked by this particular listener , uh, I have seen some of these stimulants affect men when it comes to their fertility as well. I think that there's also potential cause or a contributor to kidney stone formation in a lot of these patients because the stimulants can often be diuretics or burn off more water than cause them to be relatively dehydrated.
Speaker 2:I never thought about that.
Speaker 1:And so there's several different urologic conditions that I think can be affected by, by the stimulant. I think that the stimulant probably contributes to some inflammation in the body. Mm-Hmm . <affirmative> , which may make other conditions a little worse. I think the stimulant can also, in some patients, cause them to leak a little bit more urine. It can like be a little bit more kind of , uh, irritating to the bladder. So I, I wouldn't be surprised about a lot of those things. But for erections, the way that I kind of think about it is this, if you have erectile dysfunction
Speaker 2:Mm-Hmm. <affirmative> .
Speaker 1:And one of the reasons is because you're not focused on what you're doing.
Speaker 2:Okay. <laugh> ,
Speaker 1:You're real distracted. <laugh>, you know, I think one of those light bulbs is up .
Speaker 2:<laugh> got conditions . Oh , that's me. And , and
Speaker 1:This is actually with women, happens all the time. Like a lot of times sex therapists will argue that women are not achieving an orgasm or enjoying intercourse 'cause they're distracted, you know?
Speaker 2:Mm-Hmm. <affirmative> ,
Speaker 1:I gotta make lunch for the kids and there's a sock on the ground that somebody dropped or,
Speaker 2:Or my husband sucks <laugh> Yes. Or
Speaker 1:Something not sucks . Maybe he
Speaker 2:Doesn't . And
Speaker 1:So maybe he should. And so , uh, concentration is something that these types of stimulants are supposed to try to help with. Mm-Hmm .
Speaker 2:<affirmative>
Speaker 1:Like focusing. Uh , a lot of the techniques that sex therapists use are gonna be like these, you know, stay in the moment kind of techniques so you don't get distracted for those kinds of patients. So in particular, women, I usually notice that a lot of times when they get on stimulants, sometimes they do a little bit better.
Speaker 2:The women.
Speaker 1:The women.
Speaker 2:Okay.
Speaker 1:But when it comes to men, my experience is usually the opposite. My, my experience is that usually these stimulants end up leading to , uh, some erectile dysfunction. Now, if the men of the eighties are to be believed, <laugh> cocaine, which is a stimulant <laugh> , really helped with erections. <laugh> . But , but, but I can't, I can't speak to that. You
Speaker 2:Can't. Why are you winking at me?
Speaker 1:I'm not winking at all. You're making that up. You're more from the eighties than me.
Speaker 2:Maybe by one year. Yeah .
Speaker 1:Two
Speaker 2:Years. Okay. Maybe three.
Speaker 1:Probably in prime cocaine smoking age. You
Speaker 2:Know, it was a good time back then. So,
Speaker 1:But , um, my , uh, my analogy that I use in the clinic is that , um, being overly hyped up or overly kind of anxious. Mm-Hmm.
Speaker 2:<affirmative>
Speaker 1:Is a big killer because
Speaker 2:Oh , yeah .
Speaker 1:So like Tom Cruise, when he is in the movies jumping from building to building and escaping, like exploding planes,
Speaker 2:He is a.
Speaker 1:He does not have a. Oh , <laugh> . 'cause epinephrine when it goes up really kills the, the mood.
Speaker 2:Oh.
Speaker 1:And so there's two types of symptoms , uh, uh, two types of systems that we have in our body that, that help the , um, erections work.
Speaker 2:Mm-Hmm. <affirmative>
Speaker 1:One is called the parasympathetic system.
Speaker 2:Mm-Hmm. <affirmative> .
Speaker 1:That's usually what's good when we're resting. And the sympathetic nervous system when we're excited.
Speaker 2:Okay.
Speaker 1:And so, because I , I was a nerd <laugh> . Okay .
Speaker 2:I heard
Speaker 1:P and s parasympathetic and sympathetic point and shoot. So that's how we knew which system helped. So those of you studying for your medical boards,
Speaker 2:The pss
Speaker 1:Point and shoot. Okay ? So you have to have an intact parasympathetic system to get an erection. So if you're too riled up, you won't get the erection. Mm-Hmm.
Speaker 2:<affirmative> .
Speaker 1:But if you're not riled up enough, you won't orgasm
Speaker 2:So complicated.
Speaker 1:You can't just lie there. You understand? You gotta be in the moment a little nervous.
Speaker 2:Mm-Hmm . <affirmative> , if you're the girl, you could just lie there.
Speaker 1:And that's , uh, one reason that we use yohi being Mm-Hmm .
Speaker 2:<affirmative> ,
Speaker 1:Which is an , which is a stimulant
Speaker 2:And a supplement.
Speaker 1:And a supplement yohi being is a stimulant that helps men and women who can't orgasm to orgasm.
Speaker 2:Okay. So
Speaker 1:We use that kind of just fleetingly right before intercourse to help with an orgasm. Mm-Hmm. <affirmative> . Uh , which is something that we treat here in the clinic. A lot of patients come to us not knowing where to go if they have orgasmic problems and they're a man delayed orgasm.
Speaker 2:Mm-Hmm. <affirmative> in ,
Speaker 1:In in particular. Um, and so these types of stimulants , uh, whether they be Adderall or others, I've noticed that in , um, in, in men, even younger men can start leading to some erectile dysfunction.
Speaker 2:Hmm .
Speaker 1:So I think that if you're taking one of those and, and it's interfering with the intercourse, you need to adjust the timing, you know , um, maybe take it earlier in the morning instead of later in the day. Mm-Hmm.
Speaker 2:<affirmative> ,
Speaker 1:Um, maybe , uh, adjust the dose , uh, and speak with your, you know, prescribing medical provider to kind of optimize what you're on.
Speaker 2:Right.
Speaker 1:Um, and if you have erectile dysfunction already, don't be surprised if it gets worse. Right.
Speaker 2:And ,
Speaker 1:Um , that makes
Speaker 2:Sense. But ,
Speaker 1:But I think these medicines affect people in different ways. Some people get really anxious and really amped up. Mm-Hmm . <affirmative> . And some people don't, they get kind of calm because they're able to focus more.
Speaker 2:Right .
Speaker 1:And so I think that probably has a lot to do with whether or not that's gonna affect your erections. Hmm . Uh , and the other part of this , uh, question that the listener mentioned was that he's already using a daily Cialis to help with , um, uh, with blood flow to his penis. Mm-Hmm.
Speaker 2:<affirmative> ,
Speaker 1:I would just kind of mention that , um, uh, on this station, on the radio, if you're listening or if you've gotten any kind of ads, <laugh>, if
Speaker 2:You're
Speaker 1:Watching this on the internet , uh, you're gonna see a lot of information about shockwave therapy or linear compression therapy, or any sorts of complex words to talk about how we can rejuvenate your erectile ability . And , uh, the ideal candidate is the one who's using daily Cialis and it's working Mm-Hmm . That patient is the one that , uh, the technology that we use is, is low intensity shockwave therapy. Speaker 2:Isn't , well the guy's got scrubs on and he has no name on his scrubs. Yeah . It's just some random guy in scrubs. And I'm like, okay. That's his little red flag there guys.
Speaker 1:That's right.
Speaker 2:You wanna see a urologist or at least a professional.
Speaker 1:Yeah . So if you have erectile dysfunction, go to a place that treats erectile dysfunction, <laugh>. Don't go to a place that just sells one thing. Go to
Speaker 2:A lollipop store.
Speaker 1:Yes. And we've had a tremendous success, I mean, through the show and through kind of, kind of me harping on the same kind of thing every week, which is that Mm-Hmm . <affirmative> , you know, I know it takes a little bit of time to see us, but at least you're not gonna throw $10,000 down the drain. Right.
